The Irish Hospice Foundation Forum 2015 Photo Mosaic Project Deadline Friday, 28th August

The Irish Hospice Foundation has announced the opportunity to take part in creating an interactive photo mosaic art project at Forum 2015.  The photo mosaic will be unveiled at the end of the conference and will comprise hundreds of photos of loved ones sent in by Conference participants and the public.

To participate in the project you just need to email your photo to:  photoproject2015@hospicefoundation.ie 

When making the submission you should include the following information:

• Name of person making the submission
• Name of person in photo
• Relationship to person in photo
• Verify you are the owner of the photo or have obtained permission from the owner to submit the photo
• Any additional brief details you would like to include about the person in the picture or why you have chosen this particular photo as a tribute

Closing date for receipt of photos is Friday, 28th August.
